We love to help our clients access funding for support services.
TransitCare’s client service team members are experience in helping you understand your eligibility for funding.

Please note that the information below is general and may not be applicable for your location or your needs.
The main support for people over 65 years (or over 50 years for Aboriginal or Torres Strait Islander peoples) who need help with transport is via My Aged Care. Programs such as Commonwealth Home Support Program (CHSP) and Support at Home provide funding for community transport via a needs-based assessment.
Contact My Aged Care to get your assessment and discuss your transport needs such as attending medical appointments or getting to the shops.
Your assessment will determine how much of the cost will be funded by the government, and how much you will need to contribute.
If are not eligible for funding but still want transport support, you can still benefit from our caring and client-centred services for older people by paying us directly.

The Community Transport Program supports people who have trouble accessing transport options to get around their local community. You may have trouble accessing transport because of:
- where you live
- your financial situation
- disability, mental health, or chronic health conditions.
The services include door to door transport for appointments, shopping and social activities. You may be eligible for this program if you:
- are under 65 years of age
- have no or limited access to other transport options, including public or private transport or transport funded through other funded/subsidised schemes
- are not receiving transport services for the same or similar purpose under another government program.
TransitCare receives funding for this service in some of our locations including Brisbane South, Logan, Cairns, Townsville, Ingham, Charters Towers and Ayr. Contact our Client Service team on 07 3422 7900 to see if we offer these services in your location.
TransitCare believes people with disability should have genuine choice and control over who you travel with, and how. We offer a deeply person-centred, safety-focused transport service which supports accessibility, inclusion and social participation for people with disability.
As a registered NDIS provider, TransitCare makes sure our drivers and trip scheduling system can meet the diverse, often unpredictable needs of people with disabilities, including accessible vehicles, early-morning medical appointments, weekend social participation, urgent transport and essential daily living activities.
Funding for TransitCare’s services is usually provided as a budget for transport funding within your NDIS plan.
Getting out and about is important for your health and wellbeing but transport to social activities can be difficult.
In Brisbane South, Logan and the Gold Coast areas, TransitCare’s social activities and events help clients who receive funding through My Aged Care programs such as Commonwealth Home Support Program (CHSP) and Support at Home.
You might like to join a group outing and enjoy a day of sightseeing, shopping and eating out. Or spend a few hours in a gentle exercise class or testing your trivia knowledge.

If you are not eligible for government funding for transport or social support services, you may choose to self-fund, paying TransitCare directly. TransitCare works hard to keep our fees for transport services and social activities as low as possible. We aim to deliver a high-quality service which helps self-funded clients achieve the right balance of convenience, cost and independence.
